4 month old puppy slimy poo?

My 4 month old puppy is all of a sudden pooping out really slimy stuff. Its not runny just looks like mucus and its yellow. She has been throwing up a couple of times. Should I take her to vet or what? She hasn't changed in her behavior at all just her waste. Please help!
